Surgical orthodontics—also known as orthognathic surgery—is a specialized treatment used to correct severe jaw discrepancies, bad bites, and facial imbalances that cannot be treated with braces or aligners alone.
This type of care combines orthodontics with oral and maxillofacial surgery, one of the nine recognized dental specialties, to address complex conditions involving the mouth, jaw, face, and skull. Your orthodontist will work closely with a skilled oral surgeon to create a treatment plan tailored to your needs.
When is surgical orthodontics needed?
Surgical orthodontics may be recommended for adults whose jaws have finished growing—usually around age 16 for females and 18 for males. It is considered when:
- The upper and lower jaws don’t align properly.
- Orthodontic treatment alone cannot achieve a functional bite.
- Severe bite problems or jaw discrepancies affect chewing, speaking, or facial balance. Orthognathic surgery repositions the jaw so that braces or aligners can then align the teeth into their ideal positions.
How does the process work?
- Orthodontic preparation – Braces or aligners are used to begin aligning the teeth.
- Surgery – Performed in a hospital setting by an oral and maxillofacial surgeon. The procedure can take several hours depending on your case.
- Recovery – Most patients need about two to four weeks of rest. Time away from work or school is recommended.
- Finishing phase – Once healed, your orthodontist fine-tunes your bite with braces, typically worn for 6–12 months after surgery.
- Retention – A retainer helps maintain your new, healthy smile.
